Requirements

  • 1 to 2 years of experience as a group facilitator in Substance Abuse and Mental Health settings
  • Bachelor's Degree in human services field, preferred
  • Minimum 1 year of full-time employment working in human services or in behavioral health sciences
  • Knowledgeable of Department of Children and Families (DCF) guidelines and Joint Commission standards
  • Must be familiar with 12 Step Recovery programs
  • Knowledge of Kipu, preferred

Responsibilities

  • - Coordinate and provide care that is safe, timely, effective, efficient, equitable, and patient-centered
  • - Handle case assignments, draft service plans, review case progress and determine case closure
  • - Communicate patient needs to a variety of care team members and follow up accordingly.
  • - Manage discharge plans upon completion of treatment.
  • - Attend clinical and administrative meetings for inter-departmental sharing of information.
  • - Work collaboratively with patients, families, therapist, and referral sources to ensure high quality care
  • - Facilitate multiple care aspects (case coordination, information sharing, etc.)
  • - Maintain the patient's comprehensive clinical record through detailed documentation
  • - Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
  • - Promote quality and cost-effective interventions and outcomes
  • - Record cases information, complete accurately all necessary forms and produce statistical reports
  • - Help navigate legal issues that patients are currently facing up to and including appearance in court hearings.
  • - Adhere to professional standards as outlined by protocols, rules, and regulations
  • - Act as the patient's advocate as it relates to insurance coverage and financial assistance
  • - Continually monitor and evaluate each client’s progression through their case management plan, and develop corrective action revisions to the plan as needed
  • - Arrange abeyance with social services, health, and governmental agencies
  • - Perform other duties as assigned by the supervisor, which are consistent with the position and in compliance with agency policies and procedures.
