The Complex Case Manager advises and supports staff to initiate early discharge planning for patients with complex needs and ensures timely assessments are completed to facilitate discharge.
What actions are taken in the case of delayed discharges?
The Complex Case Manager identifies, monitors, and progresses delayed discharges and lost bed days within the patient journey and referrals, and may complete and submit the SitRep for Delayed Transfers of Care in the absence of the Head of Complex Case Management.
