Responsibilities

  • - Assist the RRRT Service Manager in the management of a designated caseload resulting from patients you have identified as appropriate for safe and timely discharge from Kingston Hospital.
  • - Lead on the development and implementation of the RRRT function of in-reach to Kingston Hospital.
  • - Improve patient access to overall HRCH NHS Trust community services to ensure we get it right the first time, every time.
  • - Work closely with RRRT Service Manager, Divisional Managers, Assistant Directors and Clinical Operational Leads to deliver the agreed outcomes.
  • - Provide leadership and representation of RRRT at Kingston Hospital Foundation Trust meetings.
