What is the main responsibility of a Physical Health Support Worker?
The main responsibility of a Physical Health Support Worker is to assist with the completion of physical health checks for service users with Severe Mental Illness (SMI) and contribute to physical health assessments and care plans under the supervision of qualified practitioners.
Will I need to travel for this role?
Yes, the post holder will need to travel across various sites within the Mid Mersey footprint, which includes St Helens, Knowsley, Warrington, and Halton.
Is there an opportunity to work independently in this position?
Yes, the role requires the post holder to use their initiative and work independently in the community, both in primary and secondary care settings.
Who will I report to in this role?
You will report any concerns or observations to the multidisciplinary team (MDT), Nurse Practitioner, and GP, as well as to qualified staff regarding the service users' mental states and risk factors.
What types of teams will I be working with?
You will be working with a multidisciplinary team, which includes practitioners in physical health and mental health services.
