Responsibilities

  • Working alongside colleagues in the multidisciplinary Home Team, including Allied Health Professionals, Social Services & Public Health Improvement.

  • Participating in the planning and delivery of care and contributing to the ongoing evaluation of care provided.
  • Maintaining accurate records and documentation in accordance with policy and procedure.
  • Engaging with patients and their families to promote health and wellbeing.
  • Participating in training and development opportunities to enhance skills and knowledge.
  • Contributing to the development and implementation of policies and procedures within the team.
  • Supporting the Home Team in meeting the needs of the community.
  • Ensuring adherence to health and safety regulations and practices.
