The main focus of the role is to assess and discharge patients presenting with frailty syndrome as part of a multidisciplinary team, ensuring timely and appropriate care.
What experience is required for this position?
Candidates should have good acute and community experience with a variety of acute medical presentations and pathologies, especially related to frailty syndrome.
Will I have the opportunity to work in different settings?
Yes, there are opportunities to work in community settings across the Frailty pathway, and the role may be either static within the Frailty Team or rotational between the Frailty Team and Care of the Elderly Inpatient wards.
Is there support for professional development?
Yes, the team actively encourages in-service training and recognizes the value of supervision and appraisal to further expand your knowledge and skills.
