Responsibilities

  • Lead & Innovate: Drive the development of care pathways across the Virtual Ward and UCR Service, ensuring seamless coordination of interventions.
  • Urgent Response Optimisation: Spearhead efforts to meet the 2-hour urgent response target, utilising virtual ward capacity and technology-enabled care for enhanced service delivery.
  • Clinical Expertise: Provide expert guidance within the Virtual Ward, offering specialist support to patients, families, and healthcare professionals, while empowering staff to deliver best practices.
  • Collaborate & Expand: Strengthen partnerships across secondary, community, and primary care. Represent the services in regional networks, contributing to shared learning and MDT discussions.
  • Innovate & Expand Services: Lead the growth of Step Up/Step Down services and the integration of tech-enabled care solutions, using QI methodology to drive pathway development.
  • Operational & Strategic Support: Assist the General Manager in shaping strategic plans, ensuring a coordinated approach to pathway services and cohesive health and social care partnerships.
  • Performance Leadership: Ensure optimal clinical and operational performance, supporting the General Manager in achieving and improving KPIs.
