Responsibilities

  • Perform complete, accurate and consistent reviews of formulary exception medication or product requests; including review of appropriate labs and supporting documentation against pharmacy clinical criteria/guideline, which also involves referral/escalation of requests, when appropriate, to clinical experts with appropriate recommendations.
  • Assist with special projects as assigned by Clinical Operations management or its partners Other duties and responsibilities as assigned
  • Leads improvement initiatives and uses analytical and critical thinking skills to implement innovative solutions to problems
  • Participation in brainstorm of project operations, assist with documentation, implementation, monitoring of projects, and high level evaluations as needed
  • Accurate ordering of products and medications on behalf of dialysis facilities
  • Maintain on-going communication with facility teammates, resolve technical processing issues, and escalate all outstanding issues including drafting written responses, correspondence, emails, and spreadsheets as required
  • Ensure that current and accurate communication with facility teammates occurs in a clear, timely and concise manner
  • Being knowledgeable and providing consistent communication of DaVita’s standardized policies and procedures for the Formulary Exception process
  • Other duties as assigned.
