Responsibilities

  • - Working as part of the team and in partnership with the child, their family / carers, you will formulate individual care packages that facilitate the smooth transition from hospital to home.
  • - You will have responsibility of managing your own caseload and coordinating services to facilitate early discharge, reduce hospital re-admissions and provide support and nursing care to children and young people with acute healthcare needs and those with long-term conditions at home.
  • - Participating in the out-of-hours telephone on-call system to support all children and families on the CCN countywide caseload you will enable admission avoidance.
  • - As an excellent communicator you will support the development of ambulatory care and promote the ethos of empowering children and their families to develop the knowledge, skills and expertise to enable them to become self-caring, independent and manage their own condition at home wherever possible.
  • - Applicants must demonstrate an ability to work autonomously and as part of a team.
